Instructions for record blocking by mail:
| 1. |
Go to www.ZabaSearch.com and type in your name to run a search. |
| |
|
| 2. |
If you have a common name, and too many results appear, try narrowing your search to a State, and/or add a city. |
| |
|
| 3. |
Once the results are displayed, print the screen using the print feature found in your Internet Browser. |
| |
|
| 4. |
Check the print out, and CLEARLY CIRCLE those results that are yours in red or blue ink. |
| |
|
| 5. |
On a separate sheet of paper, please provide ZabaSearch with your full name, address, email address, phone number and date of birth. This allows us to match your name with the names you wish to have opted-out. Please note, that ZabaSearch DOES NOT use this information for any other purposes other than to create the opt-out filter, and your letter will be destroyed. |
| |
|
| 6. |
Once you have gathered the print out and the letter, please send them together, in one envelope, to:

| |
|
| 9. |
Failure to follow any of the instructions listed above may result in your request not being processed. Please provide all the information requested. |
